Celiac disease
This ring-study concerns serological diagnostics of celiac disease. Sera should be tested for celiac disease associated (auto)antibodies. These are autoantibodies directed against endomysium (EmA) and transglutaminase-2, a.k.a. tissue transglutaminase (TGA) and antibodies against deamidated gliadine (DGPA). Results should be reported in units per ml (TGA, DGPA), including the normal limits. In addition, an interpretation of the quantitative results should be reported (positive, equivocal, negative).
For IgA antibodies against transglutaminase-2 an upper limit of normal (ULN) classification can be reported. The ULN is the upper limit of the equivocal area of the test. The classifications are: <1x ULN, 1– 3x ULN, 3–10x ULN, >10x ULN.

Details
The tests that are evaluated are:
IgA and IgG antibodies against endomysium (EmA), translutaminase-2 (TGA) and deamidated gliadine (DGPA). In regular diagnostics testing for specific IgA antibodies in individuals with detectable total IgA levels is sufficient. In IgA deficient (IgA < 0,07 g/L) individuals, IgG antibodies should be tested additionally. Only in cases of absolute IgA deficiency IgA tests can be omitted. In the ring-study we chose to test for both IgA and IgG antibodies in all samples irrespective of the total IgA level, this to be able to limit the number of samples to five per round.
TGA-IgA ULN classifications can be provided for all samples but are at this time primarily relevant for diagnosing children (Espghan richtlijn, Husby et al. JPGN 2020; 70, 141-156). Independent of complaints, a TGA-IgA level of > 10x ULN is sufficient for the diagnosis and a duodenal biopsy can be omitted, provided that the antibodies are confirmed in an independent blood sample with a positive EmA. Patients with positive TGA-IgA <10x ULN duodenal biopsies are recommended.
